This system is for the convenience and use of remote users who wish to access verbal results for NEGATIVE or POSITIVE drug screening samples. This system will allow for the entry of either the sample donor’s Social Security number or the Chain of Custody identification number on the form submitted with the sample. NEGATIVE and POSITIVE screening results will be reported by this voice response system as soon as the results are available.
Samples that have either not been received or are currently in testing will not be reported via this system. The system will tell the user that the sample is not found and to either enter the number again, in the case of an incorrectly entered number, or to call the corporate office for more information on that particular sample.
This system is available to be accessed 24 hours a day, seven days a week.
There are currently two numbers dedicated for access to the IVR system:
214-575-6716
972-235-2042
Once the system answers, the caller will be prompted for a user identification number and a password. The pound sign must follow the number and the password.
The system will tell users what menu items are available and how to enter the sample information. At any time, users can press the “*” key to return to the main menu.
is to retrieve results using the Chain of Custody number that is the 8 digit, bar-coded number in the upper right of the chain of custody form. The # sign must be pressed after the number is entered for the system to retrieve the record.
is to retrieve results using the Donor ID, usually the Social Security Number of the donor. The # sign must be pressed after the number is entered for the system to retrieve the record.
Regardless of the number entered, the system will say both the Social Security (Donor ID) and Chain of Custody number for the sample record retrieved as well as the collection, received and reported dates. Users must listen and verify that all of this information is correct before recording the results.

The actual report is the official record of the sample test results. End users of this system bear the responsibility for verifying that the Chain of Custody number, Donor ID number and collection date read by the system match the sample in question before taking action on the results reported by this voice response system.
